About Our Center

Welcome

On behalf of our team of caregivers and support staff, thank you for considering our healthcare center. We offer a full range of medical services to meet the individual needs of our residents, including short-term rehabilitative services and long-term skilled care. Working with the resident’s physician, our staff – including medical specialists, nurses, nutritionists, dietitians and social workers – establishes a comprehensive treatment plan intended to restore you or your loved one to the highest practicable potential. Our goal is to provide quality care in a comfortable, compassionate environment, demonstrating on a daily basis our guiding principle of providing Quality Through People.

Our combination of clinical skill and comprehensive rehabilitation services enables us to care for a variety of complex medical conditions. Clinical services include: IV and infusion therapy, pain management, enteral and parenteral nutrition, ostomy and tracheostomy care, diabetes management and complex wound care. Rehabilitative therapies are provided as needed by physical, occupational and speech therapists. Alzheimer’s and dementia-related services are provided by specially trained caregivers in a comfortable and secure unit.

We have designed this web site as an informational resource for you and your family. If you have any questions or concerns, please call the executive director’s office or any department listed in the site’s phone directory.

Overview

Bay View Nursing and Rehabilitation Center accommodates 170 residents. Bay View is a 2004 recipient of the American Health Care Association’s Quality Award. We provide short-term rehabilitative care, traditional long-term skilled care and Alzheimer’s/dementia special care.

Our physical, occupational and speech/language therapists are part of Kindred's Peoplefirst Rehabilitation Services division. They work with residents confronting a variety of conditions, from stroke to dysphasia to Parkinson’s disease, as well as conditions resulting from elective surgeries, multiple trauma or recovery from amputations. The rehabilitation team creates an individualized treatment program developed with the interdisciplinary team of nursing, social services, activities and dietary in consultation with your physicians. The team members’ objective is to play an integral role in helping you regain your health and reach your maximum functional ability.

Our center also includes a separate unit dedicated to the care of residents with Alzheimer’s disease and other forms of dementia. Our Alzheimer’s program is designed to celebrate the spirit of our residents. We strive to offer a comfortable and compassionate environment that emphasizes positive life experiences and active involvement in a daily routine. The staff who care for our Alzheimer’s residents are focused on meeting their special needs. We encourage family to remain integrally involved in the lives of their loved ones and offer ongoing support through individual care plan conferences and support group meetings.

Bay View also features a therapeutic activity program that includes weekly outings, educational opportunities and lively entertainment. Family and friends are encouraged to visit frequently and are welcomed to become a part of our extended family.
